Sorry if this is in the wrong section. I wasn't sure where to put it. I assume it's the correct section. Since you're asking for the location of an object in the Story levels. I've had a little Google, Wiki, and some more, search. I don't have the costume and I can't find it online. I know the enemies in the High Stakes Heist level have the slot machine heads. But these may be exclusive.

I know the costume you are talking about... hmm... seems it's more content they forgot to give away as a prize... There's actually a good handful of content in this game they seemed to forget about giving out as prizes...

I was going to post this exact same question! I was thinking that maybe it was a special decoration, seeing as though the rest of the costume is available, but I couldn't find it (I had all content except that in the 4X level). I was looking forward to getting it when I aced the level it featured in and was very surprised when I didn't get it...

Yeah it can only be used on that sackbot, you can't save it as a costume due to it being bugged. Also they mention somewhere in that level not to tweak any of the objects as they will crash create mode as you have seen already lol. However if you hit square on it before placing the objects down it doesn't seem to crash. Guess we know why they didn't give us those objects, They need some fixing!

If I had the cross controller pack I could get the costume. But I don't have them money and I wouldn't be able to use it anyways. I don't have the Vita. Edit: remove info against EULA

The Toggle Mobsta Jackpot head can be saved if you capture the sackbot as a character, change into it, and then save the costume from there
