In the 3rd map, there are 2 levels on the left side that are locked. How do I unlock them? I've completed all story missions. I've seen the end credits, so what am I missing?

Check your quest progress. Hit triangle to look at quests, completed quests move to the bottom. You can hit square while highlighting a quest to have a marker in game to guide you. I am pretty sure the levels you are speaking of are quest levels.
