All flat platforms in top-down to 3D

I was looking at the lava top-down 3d walk on wall level and it got me thinking it'd be amazing to create a huge 3d platformer with that create method. The only problem would be the fact that you can't create hills to walk up, so it'd have to be all flat surfaces. At the moment, I can only think of two ways to make hills. One, is when you tackle a 3D platformer where the player walks on the plane that crosses the 16 layers. (x,y plane where z is up) You can edit them to actually be bumpy and hilly, but look as if the hill was cut. So Sonic's 'hills'. The other, which is the player walking along the plane only on one layer (x,z plane where z is up and x is the typical 2D games' path) involves actually programming a floor detection and would take up a ton more time to implement.
