Sorry for not remembering where it is but after looking through my level items found - it is in the Elephant Temple (Great Magician levels). Should narrow things down a little.

Elephant temple in the 2 player area. Theres also some other items. FYI you should know, there are 3 large platforms, one in gas and two above. there is a swich that makes the lower ones move up and the ones that are up move down. one player must move platforms up and down while the other player jumps across. It's difficult, and you MUST have someone good at platforming.
