I know what you mean. Did you notice those diamond shape things you shoot throughout the level? One is high right at the beginning, next is on a rooftop, etc. Each of those opens one of the doors at the end, and there are five total. I've yet to find the last one that you are missing, but I think it's probably on the elevator.

Ok, the fourth level of the MGS pack on the end there are many doors. I'v opened all, but not the last. How can i open the last? Maybe this video will help. The one I missed was from the cannon. Instead of shooting straight up shoot lower to the left. YP_cTuesPJM

I can confirm that the last door is INDEED the cannon based one.
