The hat is called Cecilia's hat but I'm afraid I have no clue what level it is to be found in! Good luck tracking it down.

I think it's in Eve's Asylum. I have the hat but I can't remember where I got it from, and Cecillia sounds like an Eve'sAsylum-like name.
