LBP2 has many levels in Story mode where a vending machine-like device appears at the end, and if the player has scored a certain amount of points, a prize is given. Is it unlockable to use in Create Mode? If so, which level can I get it in?

i don't think it is. how ever you can create your own with the score sensor. someone should really replicate that score box and give it in a level.

You would have to make it from scratch. wouldnt be that hard though. Just make a vending machine, discreetly block the chute with dissolve material and then have a score sensor (or what ever you chose to supply the output) trigger the dissolve to make the machine vend. Hook up the appropriate sound effects and that should do it.
