Ive always wondered this? Is it just the most popular? or the newest-most popular? er what?

I can't remember it in full detail but. When a level is published/republished it goes up on cool pages the more plays it gets during that time, the longer it stays up there. If its a new level thats under 1 week old, it has a chance of it sticking on cool pages for 1 week. Which is why crappy challenge levels like bomb survival and losers with no life aka russlesmuscles/champion/jeriko and others abuse this like nuts and get it to stick on cool levels by making awful survival levels and republish it like there's no tomorrow, but what is hilairous is once that week is up, those awful levels almost never get played again

Cool Levels aren't "chosen". They are merely the result of a very basic algorithm that sets a calculated score based on numerical values (ratings, plays, hearts). Those numerical values are in no way indicative of a level's quality or fun factor, which is why most levels on Cool Levels aren't actually that interesting to play. In fact, the "best" way to get onto Cool Levels seems to be to make a level which somehow skews any or all of those numerical values. I have long since stopped believing in the lie that reaching the "Cool Levels" is somehow a measure of worth as a creator.
